Social Worker/Case Manager (PDS Service Advisor)

Multi-jurisdictional, planning and development agency seeking a motivated and client oriented social worker to serve as a Case Manager/Social Worker for the Medicaid Waiver programs: Participant Directed Services (PDS) and Michelle P.  Position includes home visits in the Lincoln Trail region.  Requires Bachelor’s Degree in Social Work, RN, or a field relevant to the Aging and Independent Living population.  Applicants with Bachelor’s Degree in other fields must have two years of experience working with the appropriate populations.  Specific areas of work include assessment, case management, and care planning.  Must be able to handle multiple tasks, projects, and programs simultaneously and be team oriented.  Extreme attention to detail and the ability to complete accurate paperwork/documentation is a must.  Applicant must have working computer skills.  Knowledge of human service programs required.
