Social Work is at the Heart of our CASE Team

Every March, we take a moment to celebrate the professionals who serve as the backbone of our social safety nets. While Social Work Month typically falls in March, at LTADD, the impact of our CASE (Community Aging & Social Engagement) team is felt every single day of the year.
The theme for Social Work Month this year, “Empowering Social Workers,” rings especially true for our CASE managers. Our CASE team members don’t just manage cases, they change lives, safeguard dignity, and act as the vital link between our region’s seniors and the resources they need to remain independent.
For many of our neighbors in the eight-county Lincoln Trail region, the world of social services can feel like a maze. Our CASE team serves as the expert guides. They work one-on-one with seniors and individuals with disabilities to develop personalized care plans that allow them to live safely in their own homes for as long as possible.
Their daily dedication includes:
- Comprehensive Assessments: Meeting residents where they are by visiting homes to evaluate physical, emotional, and environmental needs.
- Connecting Resources: Whether it’s arranging for home-delivered meals, other in-home services, or location of community resources, they ensure no stone is left unturned.
- Advocacy: Being a voice for those who may feel unheard, ensuring they receive the benefits and respect they deserve.
- Crisis Management: Stepping in during life’s most difficult transitions to provide a steady hand and a clear path forward
Social work in the field of aging requires a unique blend of clinical expertise and deep empathy. It’s about more than checking boxes on a form; it’s about listening to the stories of a veteran in Marion County, supporting a caregiver in Grayson County who is at their wit’s end, or ensuring a grandmother in Nelson County has the supports she needs to stay in the home she loves.
Our CASE team members are problem-solvers by trade and advocates by heart. They navigate complex regulations and shrinking budgets, yet they never lose sight of the individual at the center of the work.
The work of the LTADD CASE team is often quiet work. It happens in living rooms, over phone calls, and in the meticulous documentation that ensures funding stays in our region. But the results are loud:
- Lower rates of premature institutionalization.
- Improved nutrition and health outcomes.
- Decreased social isolation for our elderly population.
