NOW HIRING | Waiver Case Manager

The Lincoln Trail Area Development District (LTADD), a multi-jurisdictional, planning and development agency, is seeking a motivated, team-oriented and dynamic individual to serves as a Waiver Case Manager in the Community Aging & Social Engagement (CASE) Division. Position is responsible for services including, but not limited to, assistance to eligibleMedicaid participants receiving waiver services to enable them to remain in the community setting. Responsibilities include working with these individuals to identify needs, develop a plan of care, assist with arrangement for services (when necessary), monitor the provision of services and reassess the person’s needs and progress on a regular basis. Responsibilities include assistance with hiring of PDS employees and the approval process for payroll. The Service Advisor may be asked to work in other programs within the aging department based on funding streams or departmental needs.
Must meet the requirements for the Kentucky Homecare program case manager and Service Advisor in the Kentucky Participant Directed Services (PDS) projects. Bachelor’s in Social Work or related health or social services field or other acceptable degrees include Public Health, Psychology, and Sociology is required.
The region is best served with a detail-oriented individual with excellent organizational skills. Aptitude in relationship building is imperative, aiding in efficient communication across all coordinating planning agencies and local, state, and federal officials.
