CASE Division Grows with Addition of New Waiver Case Manager

At LTADD, our ability to serve the community is only as strong as the people who make up our team. We are always looking for compassionate, driven professionals who are dedicated to breaking down barriers for our region’s residents. That is why we are thrilled to welcome Tiffanie Disney as our newest Waiver Case Manager.
Tiffanie comes to us from Brandenburg in Meade County, bringing a deep, personal commitment to public service and advocacy that fits perfectly with the core mission of our aging and disability programs.
Navigating the healthcare system, state Medicaid waivers, and community resources can feel like an uphill battle for individuals with disabilities and aging adults. As a Waiver Case Manager, Tiffanie will act as a vital navigator, advocate, and support system for families across our eight-county region.
When asked about what drives her professional dedication, Tiffanie shared a message that resonates deeply with everyone here at LTADD:

When she isn’t busy connecting community members to critical care plans, Tiffanie stays grounded by focusing on family and adventure. She loves dedicating her free time to her kids and her pup, and she is always looking forward to her next travel destination.
