Program Spotlight: Maintaining Independence at Home with LTADD’s In-Home Services
For most of us, there truly is no place like home. As we age or navigate life with functional disabilities, staying in our own homes surrounded by familiar comfort, memories, and neighbors is deeply important. Yet, managing daily tasks can sometimes become overwhelming.
That is where the LTADD’s In-Home Services program comes in. Managed under the Community Aging & Social Engagement (CASE) Division, this program serves as a vital bridge, helping individuals live safely, comfortably, and independently in their own homes for as long as possible.
Our In-Home Services program provides personalized, hands-on assistance to individuals who need an extra layer of care to manage daily life. Rather than moving into an institutional or long-term care facility, participants receive tailored support right at their doorstep.
Every participant is partnered with an LTADD Case Manager who coordinates an individualized care plan, matching them with local service providers to assist with core daily activities.

| Homemaking & Chores | Help with light housekeeping, laundry and grocery shopping. |
| Home Delivered Meals | A nutritious meal delivered directly to the home for individuals who cannot cook for themselves. |
| Respite Care | Providing temporary relief and a much-needed break for primary family caregivers. |
| Escort Services | Personalized transportation and accompaniment to critical medical appointments. |
The goal is to reduce the risk of premature institutionalization by stepping in with minor, everyday supports that make a massive difference in quality of life.
To ensure these vital resources reach those who need them most, eligibility is generally focused on individuals who meet the following criteria:
- Age: Individuals aged 60 and older.
- Location: Must reside within the eight-county Lincoln Trail region (Breckinridge, Grayson, Hardin, LaRue, Marion, Meade, Nelson, and Washington counties).
- Functional Need: Individuals who are at risk of institutionalization and require assistance with Activities of Daily Living (ADLs)—such as bathing, eating, or transferring—without substantial human help.

- Complete the initial screening over the phone. Speak with an intake specialist who will ask standard questions regarding your or your loved one’s daily living needs, location, and current support network.
